GHAZ Student-Athlete Transportation Release Form
For students to use the new school vans, we must have a release form on file. Please download this form and make sure it is turned into Coach Conklin at the start of the year.

These Forms are required for ALL Athletes. 1. Please download the forms. 2. Fill out all information, make sure both the student and parent have signed each form. 3. See a Physician for a physical, and bring the paperwork with you, so that they can fill it out. Each Athlete must have an updated physical each year to participate in Athletics.
