PLEASE REGISTER EACH STUDENT INDIVIDUALLY.

1. Fill out the registration form and let us know what sport your child will be playing this season.  Fill out this form to start registration.

2. Make sure you have a current AIA Physical on file. 

The physical must be dated after March 1, 2023 to be accepted. If you do not have a current physical on file, download the forms below, make sure you have a doctor sign and date the necessary forms, and turn them into the athletic office.

3. Turn in your AIA physical and paperwork to Coach Conklin or Coach Jarman or upload a PDF of all paperwork through the registration form.

We are no longer using Big Teams For Registration, so please send the packet of paperwork to school with your student, and hand to Coach Conklin or Coach Jarman BEFORE tryouts, OR you can upload the form via this registration form.

Please make sure to fill out ALL forms in the AIA packet, including the Concussion Annual Statement and Acknowledgment Form and the Consent to Treat form.

Physicals must be resubmitted annually for each year.

4. Pay the athletic fee via configio.

Submit the athletic fee. Visit Conigio for pricing for each sport & to Pay for Sports by debit/credit card online, or you may submit cash or check (made out to Arete Prep) to the front office. 

HIGH SCHOOL ONLY

If not previously completed, every high school athlete must complete the AIA Brainbook and Opiod Courses The AIA will notify Arete electronically once the student has successfully completed the course. High school athletes are required to complete this course only once during their high school career. 

All items must be completed and turned in before your student may participate in their sport.

IF YOU DO NOT HAVE YOUR PHYSICAL TURNED IN, YOU CAN NOT PARTICIPATE IN TRYOUTS.
